Required Forms for Coaches & Managers

Complete and submit:

Manager & Coach Application Form
Little League Volunteer Application
A copy of a Govt. issued ID (Driver's license or Military ID).

Effective in 2007, the local league must conduct a nationwide search on each volunteer that contains the applicable government sex offender registry data.
